Discussing the Daily Plan®

The patient is the only constant in the health care system who is always present and yet least likely to be included as a resource. The Daily Plan® changes that!

The Daily Plan® enhances patient safety by involving you in your care. We use a single document that outlines what you can expect on a specific day of hospitalization, which a provider will discuss with you. The Daily Plan® is essentially a road map that lets you see, in black and white, what to expect on a particular day.

Relevant items can include:

  • Diagnostic tests
  • Medications
  • Nutrition
  • Appointments
  • Allergies

The Daily Plan® encourages you and your family to better understand your care by asking questions if something seems different than planned. This allows you to become an active member of our health care team.
