The Daily Plan

You, the patient are the only constant in the health care system who is always present and yet least likely to be included as a resource. The Daily Plan® can change that!

Patient and health care provider discuss patient's Daily Plan®

Discussing the Daily Plan®

The Daily Plan® enhances patient safety by involving you in your care. We use a single document that outlines what you can expect on a specific day of hospitalization or outpatient visit, which a provider, such as a nurse or physician will discuss with you. The Daily Plan® is essentially a road map that lets you see, in black and white, what to expect on a particular day.

The Daily Plan mayinclude:

  • Diagnostic tests
  • Medications
  • Nutrition
  • Appointments
  • Allergies
  • And more...
